We want to make your patient experience easy and comforting. If you are a CMC patient, you will be required to register. This creates a unique account number and identification bracelet that will help us properly identify you throughout you’re your visit. By pre-registering for your inpatient or outpatient appointment, we can help ensure a smoother process at your appointment time.

In-patient Admission Information

Every visit to Catholic Medical Center requires a registration. This registration creates a unique account number for your visit as well as the identification bracelet that you will wear for proper identification during your visit. Our preference is to pre-register all appointments to help make the patient experience smoother when you arrive. Our admissions registrar will also obtain proper insurance authorization for your inpatient stay from your insurance company, prior to your arrival.
 

Admissions Paperwork & Pre-registration

In most cases, pre-registration is completed before your arrival to the hospital. The admissions registrar will contact you by phone to obtain the necessary demographic and financial information and answer your questions. We will put this information in our computer system and create a unique account number for your visit. This also will produce an ID band which will you will wear upon your arrival for proper identification while you are here.

Pre-registering means, on the day of your appointment, you can report directly to the department you are visiting without delay. Many patients find taking care of this process ahead of time also provides peace of mind. Should you be unable to pre-register, your paperwork will be completed upon your arrival for your appointment.
 

Outpatient Registration Process

In most cases, pre-registration is completed before your arrival to the hospital. The admissions registrar will contact you by phone to obtain the necessary demographic and financial information and answer your questions. We will put this information in our computer system and create a unique account number for your visit. This also will produce an ID band which will you will wear upon your arrival for proper identification while you are here. If we are unable to reach you prior to your appointment, we will complete all necessary paperwork when you arrive.  Please allow extra time for this process.
