How can I find out if someone is a patient at CMC?
We cannot provide patient information online. To inquire about a patient at CMC, please call: (603) 668-3545. Please keep in mind if someone chooses to be a “privacy patient,” our team will be unable to share any information.
I have a condition and would like medical advice. Can you provide advice via email?
Catholic Medical Center does not provide medical advice via email. We encourage you to discuss any medical concerns with your primary care doctor. If your condition is an emergency, please call 911. If you need a primary care doctor, please call our Primary Care Access Line at (603) 314-4750 Monday through Friday. To view our doctors, use the Find a Doctor tool.
Can you help me with my prescription?
Please contact the doctor listed on the current prescription label with any questions and to request refills. You can find an alphabetical list of Catholic Medical Center's practices and contact information on this page.
Can I get my prescription delivered before I am discharged from the hospital?
Yes. For your convenience, we partner with Rite Aid to provide a Meds-to-Beds service that delivers medications for FREE. We can deliver them to your hospital room from 9:00 am until 8:00 pm, to the Discharge Hospitality Lounge or to your home Monday through Friday from 9:00 am until 12:00 pm. Please ask your nurse about the Meds-to-Beds form.
How can I find a doctor?
Our Find a Doctor online tool is a database of all doctors currently on staff or with privileges at Catholic Medical Center. If you need a primary care doctor, please call our Primary Care Access Line at (603) 314-4750 Monday through Friday.
How can I submit a complaint or compliment?
We greatly value your feedback. If you wish to convey a compliment or express a concern regarding your care at Catholic Medical Center, please ask to speak with the department director.
You may also contact the patient liaison in our Patient Experience department at (603) 663-8020 Monday through Friday. How can I request copies of my medical records?
Due to privacy practices, medical records cannot be requested online. You must complete and submit an Authorization to Release form to our Health Information Management/Medical Records department. This form, and additional information about how to request copies of your medical record, can be found by visiting our Medical Records page. You may also call the office at (603) 663-6516 Monday through Friday.
How do I obtain a copy of my X-ray/CT scans/MRI?
Contact our Radiology Imaging department at (603) 663-6449 or register for our Patient Portal and download an Authorization Release form
How do I cancel or reschedule an appointment?

We do not provide copies of birth or death certificates. Please contact the New Hampshire Division of Vital Records Administration at (603) 271-4650.
